Hope House Treatment Center is seeking an entry-level, proactive Case Manager to assist patients on their recovery journey. You will help individuals access community resources, coordinate discharge and aftercare plans, and advocate for services to promote sustained recovery.
Working with an interdisciplinary team, you will document progress, navigate housing, employment, healthcare, and benefits, and ensure timely referrals while maintaining compliance with standards and regulations.
