Family Advocacy Intervention Specialist

Choctaw Global seeks a Family Advocacy Intervention Specialist to provide trauma-informed support to at-risk children and families. In this role, you will conduct assessments, develop safety and intervention plans, coordinate services, and facilitate referrals to community and tribal resources. You will collaborate with schools, social services, and healthcare partners, maintain accurate case documentation, and deliver education on prevention and resiliency. Responsibilities

• Conduct comprehensive assessments of children and families to identify risks, needs, and protective factors.
• Develop and implement individualized intervention and safety plans using trauma-informed practices.
• Provide ongoing case management, advocacy, and follow-up to ensure access to appropriate services and support.
• Coordinate care with tribal, community, school, healthcare, and social service partners.
• Respond to crisis situations, de-escalate risk, and connect families to urgent resources as needed.
• Maintain timely, accurate, and confidential case notes, reports, and required documentation.
• Educate families and community partners on prevention, healthy relationships, and resiliency-building strategies.
• Participate in multidisciplinary team meetings and case staffing to review progress and adjust plans.
• Support Choctaw Global's mission and cultural values in all client and community interactions.
• Engage in ongoing training and professional development to strengthen advocacy and intervention skills.
