** Responsibilities:*
* * Provide high-quality patient care to a diverse population of residents, including those with complex medical conditions and rehabilitation needs
* Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team
* Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
* Monitor and report patient progress, and modify care plans as needed
* Collaborate with other healthcare professionals to ensure seamless transitions and comprehensive care
