Socket.dev is hiring a Hospice Registered Nurse Case Manager in Ohio. The role involves traveling to patients' homes and facilities to provide comprehensive assessments, develop care plans, and coordinate hospice services with the IDG and families. The schedule is Monday–Friday, 8:00 AM–5:00 PM.
Qualified candidates are licensed RNs with BLS, comfortable with home and facility settings, and able to travel with valid driver's license and auto insurance. Hospice experience is preferred.
