Care 360 Hospice is seeking a Hospice Registered Nurse Case Manager to join our Ohio team. You will travel to patients' residences, perform assessments, develop care plans, and coordinate hospice services with the IDG to meet patient and family needs.
The role requires an active Ohio RN license, BLS certification, at least 1 year of clinical experience, and a valid driver's license with reliable transportation. Hospice experience is preferred; schedule Monday-Friday, 8:00 AM-5:00 PM.
