Responsibilities:
Provide direct patient care, including assisting with activities of daily living, such as bathing, dressing, and grooming
Monitor and report changes in residents' conditions to the nursing staff
Assist with mealtime, including setting up trays and feeding residents
Maintain a clean and safe environment, adhering to infection control protocols
Document patient information accurately and thoroughly
Collaborate with the interdisciplinary care team to develop and implement individualized care plans
Participate in ongoing training and education to stay up‑to‑date on industry best practices and regulatory requirements
