Licensed Practical Nurse; LPN

Job Summary

Under the supervision of a Registered Nurse or Physician, provides nursing care to patients, including data collection, monitoring, documentation, and patient/family education. Performs licensed patient care functions and ensures confidentiality of medical records.

• Delivers individualized patient care aligned with the care plan, addressing physical, emotional, cultural, spiritual, psychosocial, and educational needs.
• Monitors and documents patient status such as vital signs, dressings, drains, and lab results.
• Communicates observations and patient responses to the healthcare team and notes changes in condition.
• Maintains accurate, confidential medical records.
• Collects and documents delegated data using specialized equipment and orients patients and families to routines.
• Administers and documents medications following policies.
• Performs additional duties as assigned.

• Completion of an accredited LPN program recognized by the State Board of Nursing.
• At least one year of LPN experience or completion of a recognized residency or transition program in lieu of experience.
• Current LPN license recognized by NCSBN or the state of practice.
• Ability to obtain RN licensure if applicable, with immediate notification upon licensure achievement.
