Home Care Registered Nurse (RN) – Mercy One/Trinity Health Home Care, Mason City

Shift Monday - Friday 8am-4:30 pm;
Weekend rotation based on how many nurses are employed. On call only Saturday and Sunday 8am-4:30 pm on your scheduled weekend. No weekday or weekend evening call.

Mercy One/Trinity Health Home Care in Mason City is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home care team. In this rewarding role, you’ll deliver high-quality, personalized care to patients in the comfort of their homes, helping them maintain independence and improve their quality of life.

• Perform comprehensive patient assessments and identify care needs.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Administer medications and treatments, monitor patient responses, and adjust care as needed.
• Educate patients and families on disease management, medications, and wellness strategies.
• Monitor vital signs and manage chronic conditions with timely interventions.
• Accurately document all care activities and maintain compliance with healthcare regulations.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and guidance to patients and their families.
• Participate in ongoing education and quality improvement initiatives.

• Active RN license in the state of Iowa.
• BSN preferred; ADN accepted.
• Previous experience in home health or community nursing is a plus.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license and reliable transportation.
