Position SummaryThis position provides direct care to patients under the direction of the Registered Nurse (RN). Delivers assigned, delegated, direct care consistent with the individualized treatment plan. Performs routine care consistent with established standards policies and procedures. Maintains a safe and therapeutic treatment environment.
Major Duties and Responsibilities- Collects & reports health data
- Gathers objective data & observations about the patient
- Differentiates between normal & abnormal observations
- Reports changes in patient status or abnormal findings to RN
- Implements established plan of care for each patient
- Understands the plan of care for each assigned patient
- Completes interventions identified on the plan of care
- Delivers delegated aspects of direct care
- Provides care based on the established treatment plan
- Involves patient & involved others in the care process
- Conducts designated groups within the program based on the curriculum
- Engages in individual & purposeful interactions with patients & involved others
- Assists patients with physical cares as needed
- Completes vital signs, weights, & EKG strips
- Transporting patients to appointments and other activities as needed
- Monitors patient & environmental safety continuously
- Rounds on patients at the designated times
- Maintains a calm, healing environment
- Checks belongings for restricted items; secure dangerous objects
- Documents in the electronic medical record (EMR)
- Completes nursing shift summary on assigned patients
- Reflects individual patient’s progress towards treatment goals
- Articulates interventions utilized and responses to the interventions
- Notes are factual and concise
- Obtains RN co-signature for EMR notes
- Demonstrates understanding of current therapeutic techniques
- Continuously participates in learning opportunities to expand inventory of intervention options
- Applies learning & experience to patient care
- Effectively responds to escalating behavior
- Observes patients for risk of aggressive behavior
- Demonstrates an ability to identify early changes in behavior that indicate escalation
- Engages with patients who are exhibiting early behavioral changes
- Applies alternatives to physical intervention
- When physical intervention becomes necessary, implements appropriately
- Provides information to patients & families
- Presents information to patients & involved others utilizing age specific principles
- Reviews comprehension of information
- Identifies issues of customer service
- Acts to resolve customer service issues within available options
- Refers complex & unresolved issues to the charge nurse or manager
- Initiates emergency action when indicated
