As a Hospice Admissions RN, you play a critical role in guiding patients and families through the start of their hospice journey:
- Conduct pre-admission evaluations to determine eligibility and appropriateness of care
- Educate patients and families about hospice services with compassion and clarity
- Obtain informed consent and initiate the admission process
Perform comprehensive nursing assessments and develop individualized care plans - Deliver interventions that maximize comfort, dignity, and quality of life
- Graduate of an accredited RN program
- Active Registered Nurse license in the state of practice
- Current CPR certification
- Hospice experience (preferred, not required)
- Strong clinical judgment and ability to work independently
- Compassionate communication skills with patients, families, and care teams
- Understanding of quality assurance and patient care standards
