Key Responsibilities- Provide direct personal care to hospice patients in home settings
- Assist with activities of daily living ADLs including bathing grooming oral care dressing and toileting
- Support patient comfort and safety including proper use of medical equipment oxygen bed rails etc
- Perform skin care and positioning to prevent breakdown and promote comfort
- Assist with meal preparation feeding and light household tasks as assigned
- Take and document vital signs temperature pulse respirations blood pressure as directed
- Observe and report changes in patient condition to the RN Case Manager
- Provide emotional support and companionship to patients and families
- Offer respite care for caregivers when needed
- Maintain accurate timely documentation in compliance with hospice standards
- Minimum 16 years of age
- Active State Tested Nursing Assistant STNA
- Certified Nursing Assistant CNA certification
- Listed on the state nurse aide registry in good standing
- Current BLS
- Passionate care to terminally ill patients
- Strong communication and observational skills
- Prior experience in hospice home health or long term care
- Experience supporting patients with chronic or terminal conditions
