Registered Nurse RN Hospice Monday Friday Day Shift Location Southwest Ohio Schedule Monday Friday 800 AM 500 PM Only 2 holiday on call days per year

• Manage a patient caseload and lead the full nursing process assessment admission care planning implementation and evaluation
• Deliver holistic patient care addressing physical emotional psychological social and spiritual needs
• Collaborate with the interdisciplinary care team including physicians social workers and chaplains
• Coordinate care with attending physicians and external providers
• Conduct routine patient visits and update individualized plans of care
• Provide end of life care including clinical pronouncement and family support
• Maintain accurate timely documentation in compliance with state federal and company standards
• Supervise and oversee care delivery by Home Health Aides HHAs

• Active Registered Nurse RN license in Ohio or compact state if applicable
• Graduate of an accredited nursing program
• Current CPR and First Aid certification
• Knowledge of quality assurance and hospice care standards
• Valid drivers license and auto insurance
