Description:

Full-time position, Monday to Friday. No on-call or weekend requirements. Patient care will involve visiting clients in their homes. Nurses will provide direct care, assessment, and support. Collaboration with interdisciplinary teams is essential. A focus on patient-centered care and outcomes is expected. Nurses will document patient care accurately. Experience with charting systems is preferred. Candidates should be prepared to manage a variety of patient diagnoses.

Candidates should have strong communication and organizational skills. This role requires a commitment to quality patient care and safety.
