Hospice LPN Night

Join Addus Family company as a Hospice LPN on our night shift in Fayetteville. Provide compassionate end-of-life care in patients' homes, performing assessments, medication administration, pain and symptom management, and wound care. Collaborate with RNs, physicians, social workers, and chaplains to execute individualized care plans. Document accurately in EHR, follow strict compliance and infection-control standards, and support families with education and emotional guidance.

Responsibilities

• Provide in-home hospice nursing care on night shift
• Perform focused assessments and monitor patient status
• Administer medications and manage pain and symptoms
• Provide basic wound care and comfort-focused interventions
• Document care and assessments in EHR accurately and timely
• Follow hospice protocols, safety, and infection-control standards
• Coordinate with RNs, physicians, and interdisciplinary team members
• Educate and support patients and families on disease process and end-of-life care
• Respond to urgent changes in patient condition during night hours
• Participate in ongoing training and quality/compliance initiatives
