Mason Health Care Center, a boutique skilled nursing facility with a strong leadership team and low agency usage, is seeking a compassionate and experienced Social Services Director to lead our Social Services Department.
This is a key leadership role responsible for supporting residents and families while ensuring compliance, care coordination, and high-quality psychosocial services.

The Social Services Director plans, develops, and directs a comprehensive social services program to support residents’ emotional, social, and discharge planning needs while ensuring compliance with all state and federal regulations.
Key ResponsibilitiesLead and manage the Social Services Department
Complete resident assessments and develop individualized care plans
Provide counseling and support to residents and families
Coordinate discharge planning and community resources
Participate in interdisciplinary care plan meetings
Ensure compliance with all federal and state regulations
Maintain accurate documentation and social service records
Advocate for residents’ rights and psychosocial well-being
Collaborate with nursing, therapy, and administration teams
Bachelor’s Degree in Social Work or related field required (MSW preferred)
LSW or LMSW (preferred or required based on state guidelines)
Previous experience in a skilled nursing facility strongly preferred
Prior leadership or supervisory experience preferred
Strong knowledge of long-term care regulations
Excellent communication and interpersonal skills
