A Day in the Life of this Role:

Reporting to the Site Medical Director, the Primary Care Physician at Outer Cape Health Services (“OCHS”) provides comprehensive and continuing care to patients requiring routine and emergency treatment at health centers or other locations within OCHS’s scope or practice including skilled nursing facilities. Additionally, the incumbent refers patients to other providers as appropriate and provides services in support of health promotion and preventive medicine.

Summary

of Duties:
  • Lead and collaborate within a Team Based Care model (including RNs, LPNs, Medical Assistants, Behavioral Health Clinicians, and Patient Access staff) to manage the health of a patient panel and champion shared accountability for outcomes.
  • Maintain an assigned patient panel and meet access and quality expectations while balancing continuity, and team-based care principles.
  • Participate in and/or lead daily team huddles to anticipate patient needs, identify care gaps, and coordinate workflow.
  • Provide clinical oversight, mentorship, and support to nursing and allied health staff to ensure team members work at the top of their licensure.
  • Collaborate with Integrated Behavioral Health providers and Community Health Workers to address social determinants of health and mental health needs.
  • Support same-day access, urgent care needs, and flexible scheduling strategies to meet patient demand.
