Job Description
Join us! Going strong since 1974, CASE is an educational collaborative agency made up of 11 regional and municipal districts affording member and non‑member school districts the opportunity to augment local services by planning, developing and implementing programs for students with special needs.
This position reports to the Program Administrator.
JOB GOAL:
  • To provide guidance to CASE in all areas related to transition planning;
  • To implement transition services for CASE students;
  • To work collaboratively with parents/guardians and colleagues to meet the academic, social/emotional, and behavioral needs of students with disabilities.
PERFORMANCE RESPONSIBILITIES:
  • Will participate in the creation of transition plans for all students 14 years or older identifying student needs and appropriate transition activities and services.
  • Will collaborate with Teams to create or update all aspects of the IEP related to transition planning to meet the student’s post‑secondary goals.
  • Will perform vocational and transition assessments for specific students (i.e. three‑year reevaluations, as requested by the district) to inform the IEP and team process.
  • Will participate in Team reevaluation and annual review meetings for students aged 14 and above.
  • Will coordinate the school’s community work program and develop employment opportunities for students in the community.
  • Will provide consultation to classroom teachers to ensure community trips are centered on students’ IEP and transition goals.
  • Will integrate MA Career/Vocational Technical Education Frameworks into content across the curriculum.
  • Will plan and maintain a transition curriculum to be implemented through the classrooms with the support of classroom teachers and instructional aides/job coaches.
  • Will select appropriate instructional transition programs and methodologies. Plan curriculum and implement instruction that addresses students’ IEP goals and objectives and identified accommodations.
  • Will coordinate with districts, state agencies, and local service providers in regards to the status of Chapter 688 referrals and to ensure seamless transitions to adult services and supports.
  • Will keep abreast of state and federal transition service mandates and laws and services offered and available through state agencies (DDS, DMH, MRC. etc.).
  • Where appropriate, will work with member districts’ guidance departments for consultation and use of resources.
  • Will complete Age of Majority paperwork associated with the IEP in collaboration with districts and families.
  • Will assess and maintain data on the progress of all students on caseload and differentiate student goals and instruction accordingly.
  • Will write progress reports for students according to the schedule set by the host school, Collaborative, and special education regulations that accurately reports information on student progress as defined by IEP goals and objectives.
  • Will develop and implement schedules for students’ transition services in accordance with their IEPs’ service delivery.
  • May supervise and facilitate student activities during non‑academic periods (i.e., a break, lunch, P.E.).
  • Will participate actively in staff meetings offering perspectives and ideas related to transition on student and program interventions.
  • Will consult with parents/guardians, teachers, counselors, therapists, consultants, and students’ developmental, emotional and social needs.
  • Will attend Collaborative‑wide and specific program staff meetings as scheduled by the Program Administrator and/or Executive Director.
  • Will keep abreast of the latest trends, materials, research, and best practices related to your field.
  • Will participate in professional development opportunities to increase knowledge base and skills, as well as maintain licensure.
  • Will maintain student and family confidentiality and abide by student records regulations.
