Home Health Physical Therapist - per visit

Overview

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a weekend position in Fayville, Massachusetts. The role offers a $5,000.00 sign-on bonus, flexible per diem, part-time or full-time schedule, and premium pay and benefits. The position will serve adult and geriatric patients in their homes and in surrounding towns including Marlborough, Stow, Acton, Boxborough, and nearby areas. Prior home health experience is strongly preferred but not required.

As a home health physical therapist, you will be an integral member of a multidisciplinary team providing skilled nursing and rehabilitative care to help clients maintain independence.

• Make visits to clients in their homes
• Perform diagnostic tests and measurements (mobility, range of joints, transfer status, stability, ambulation patterns, strength, endurance, balance, and safety assessments)
• Develop and implement individualized care plans, including manual therapy, gait training, balance and other interventions
• Continuously assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan as appropriate
• Educate clients, family members, or other representatives in rehabilitative care to promote health, safety, and independent living
• Accurately document observations, interventions, and evaluations using the appropriate electronic documentation tools

• A current license as a Physical Therapist in Massachusetts
• A minimum of one year of recent clinical experience as a licensed Physical Therapist

Base Pay: $64.00 - $72.00 per hour (or per point) depending on qualifications, plus weekend differential.
