Home Health Physical Therapist - per visit

Overview

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a job in Hopedale, Massachusetts. This position offers a $5,000.00 sign-on bonus and flexible per diem, part-time or full-time schedule with weekend opportunities.

• Make visits to clients in their homes
• Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, gait/ambulation patterns, strength and endurance, balance testing and safety assessments
• Develop and implement individualized care plans, including manual therapy exercises, gait training, balance and other interventions
• Continually assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan, as appropriate
• Educate clients, family members or other representatives in rehabilitative care to promote health, safety and independent living
• Accurately document observations, interventions and evaluations using a touch pad tablet

• A current license as a Physical Therapist in Massachusetts
• A minimum of one year of recent clinical experience as a licensed Physical Therapist

Locations:
