Direct Support Professional II - Service Coordinator ASPiRE!, an Affiliate of Seven Hills Foundation

ASPiRE! is seeking a compassionate and organized Direct Support Professional II – Service Coordinator to join our team. This role will provide direct support to individuals, assist with personal care and goal achievement, and work closely with a multidisciplinary team to implement individualized care plans. As a Service Coordinator, you will oversee participant services, coordinate care, and ensure that each person’s needs are met in alignment with their goals.

Pay: $21.50/hour

Responsibilities
  • Act as Service Coordinator for all adult day health individuals on site, ensuring their needs are met through coordination of care.
  • Oversee the coordination of medically-related physical, mental, and behavioral service needs for individuals.
  • Refer participants and families to community services as appropriate.
  • Provide supervision and guidance to interns, students, and volunteers.
  • Assess, plan, coordinate, implement, monitor, and evaluate services for each participant.
  • Participate in the comprehensive assessment and quarterly reviews of care plans.
  • Assist individuals with personal care activities, transportation, and other needs as required.
  • Perform required training and documentation for goal achievement and progress tracking.
  • Coordinate transportation and transport participants as assigned.
  • Assist the Program Manager with operational tasks, including attendance and billing documentation.
  • Complete other duties as assigned.
Qualifications

Education & Knowledge

  • Bachelor's degree preferred.
  • 1–3 years of related experience.
  • Standard driver’s license (Class C or

    D); no professional license required.
  • CPR/First Aid preferred.

Skills & Knowledge:

  • Activity Planning:
    Design engaging and educational activities to meet individual needs.
  • Behavioral Guidance:
    Provide positive support to individuals in managing behavior.
  • Case Formulation:
    Develop treatment plans based on comprehensive assessments.
  • Classroom Management:
    Maintain a supportive and organized learning environment.
  • Field Trip Planning:
    Organize educational and recreational outings.
  • Health & Safety:
    Ensure a safe and healthy environment for all participants.
  • Advocacy:
    Champion the rights and interests of individuals with developmental disabilities.
  • Behavior Management:
    Use positive behavioral interventions to address challenges.
  • Documentation & Reporting:
    Accurately document services and changes in care plans.
  • Familiarity with Seven Hills Foundation Policies & Procedures and regulatory requirements. Ability to treat individuals with dignity and respect in an age- and culturally-appropriate manner.
