Reports to:
Chief Clinical Officer
Provide clinical care coordination, community outreach, program development, and operational management of services to seniors as part of the CORE (Counseling and Outreach for the Elderly) team.
Job Duties (Essential Job Functions)- Direct Client Care:
Provide concrete, supportive, and direct case management services to elders in their home and in the community, including housekeeping, budgeting, daily living organization, communication skills, transportation, appointment management, medical information coordination, and resource identification. - Work in collaboration with other staff members to complete and review standardized Health Related Social Needs screenings.
- Provide clinical services to elders in their homes or in the community who struggle with behavioral health concerns (20 hours/12 Billable Units).
- Document all encounters in EHR.
- Outreach and Program Development:
- Attend and participate in coordination and networking meetings that support elders.
- Work with collateral providers and build and maintain relationships with community partners.
- Actively seek funding opportunities for program sustainability.
- Complete all required documentation for clinical records and administrative statistics.
- Provide direct supervision for case managers.
- Complete payroll approval and performance management of direct reports.
- Communicate and collaborate with supervisors effectively and seek guidance when situations arise.
- Compliance with all grant and licensing requirements.
- Clinical skills, knowledge, and abilities.
- Knowledge of case management principles and implementation of concrete and supportive services.
- Knowledge of DEI principles.
- 2 to 5 years of relevant work experience (preferred).
- 2 to 3 years of supervisory experience (preferred).
- Ability to work with multiple client populations, particularly families, children, and elders.
- Master’s degree required in Social Work, Psychology, or Mental Health.
- A valid Massachusetts driver’s license.
