Participant Support & Engagement

• Support individuals in daily activities that foster independence, skill development, and community inclusion.
• Assist participants in achieving personal goals identified in their Individualized Service Plans (ISPs).
• Provide guidance in social, communication, and self‑advocacy skills.
• Accompany participants on community outings and encourage participation in recreational, volunteering and vocational opportunities.
• Safely provide transportation to and from the program center and community activities in a professional and courteous manner, ensuring the comfort and safety of all passengers. This may involve the use of a personal vehicle when a KDC vehicle is not available.

Health, Safety & Compliance

• Ensure the health and safety of participants by following all agency, state, and federal guidelines.
• Support individuals with personal care needs, as required (e.g., assistance with mobility or hygiene).
• Monitor and report any incidents, behavioral changes, or safety concerns to the Program Team Lead.
• Maintain accurate and timely documentation, including attendance records, progress notes, and incident reports.

Collaboration & Communication

• Work collaboratively with the Program Team Lead and other staff to provide consistent, high‑quality support.
• Communicate professionally and respectfully with participants, families, guardians, and community partners.
• Participate in team meetings, training, and professional development activities.
• Perform other job duties as assigned.

• High school diploma or equivalent.
• Prior experience supporting individuals with developmental or intellectual disabilities or in a similar human services role.
• Strong interpersonal skills and the ability to build positive, respectful relationships.
• Ability to follow individualized plans and provide person‑centered support.
• Ability to complete and maintain CPR/FA and Safety Care certifications.
• Valid driver’s license and reliable transportation.

This position is community‑based and requires travel to homes and community settings. Candidates must be comfortable working in a variety of environments and utilizing technology for documentation and communication.
