The Case Manager is a vital member of the Regard Recovery System of Care. Working under the supervision of the Senior Therapist, the Case Manager advocates for clients, addresses immediate needs, coordinates services within community resources, and helps ensure the safety and well-being of clients and their environment.
Qualifications Education- High school diploma or GED required;
Bachelor’s degree preferred - No felony or substance abuse convictions within the past five (5) years
- If in recovery, must have a minimum of two (2) years of continuous sobriety prior to hire and active participation in a self-help program
- At least one (1) year of experience in healthcare, preferably in mental health and/or substance use treatment
- None required
- Strong verbal and written communication skills in English
- Ability to work independently and adapt to a fast-paced, changing environment
- Compassionate and client-centered approach
- Effective time management and organizational skills
- Clear and professional telephone communication

- Complete case management treatment plans within 48 hours of client admission
- Coordinate care with primary therapists and interdisciplinary teams
- Arrange and facilitate client transportation to approved medical appointments
- Assist clients with employment-related documentation, including disability and FMLA paperwork
- Communicate with legal entities (e.g., probation officers, courts, judicial staff) to support informed discharge planning and reduce legal risks
- Develop and coordinate aftercare plans based on clinical recommendations, including referrals to:
- Intensive Outpatient Programs (IOP)
- Psychiatrists
- Therapists and other providers
- Participate in daily staff meetings and weekly discharge planning meetings
- Communicate client concerns and program issues to supervisor promptly
- Complete and submit incident reports and other required documentation in a timely manner
- Adhere to ethical guidelines in accordance with applicable state regulations
- Demonstrate flexibility and adaptability in a dynamic work environment
