Job Summary:

The Physical Therapy Assistant (PTA) in this home health role provides patient care through home visits, focusing on therapy services for adult patients. The position involves coordinating patient treatment plans, documenting progress, and traveling within designated territory to support patient recovery and mobility.

Responsibilities:

• Administer therapeutic interventions and assist patients with prescribed exercises to promote mobility and functional independence.
• Complete timely and accurate documentation of patient progress within 24 hours of each visit.
• Travel up to 75 miles within the territory to conduct home visits, managing a workload of 5-6 patients per day.
• Collaborate with healthcare team members during periodic office meetings to update on patient status.
• Follow agency protocols for patient care, safety, and documentation standards.
• Maintain reliable transportation, valid driver’s license, and agency-required liability insurance at all times.

Qualifications:

• Hold a current license as a Physical Therapist Assistant in good standing.
• Minimum of 1 year of recent adult home health therapy experience.
• Possess valid CPR and BLS certifications.
• Demonstrate proficiency in the use of EMR systems such as HCHB or Pointcare.
• Must be comfortable traveling 50-75 miles daily and accepting all scheduled patients regardless of distance.
• Ability to work independently and maintain professional standards.
