Navigator Homes of Martha’s Vineyard is seeking a compassionate, motivated, and experienced Physical Therapist (PT) to join our interdisciplinary care team. We are currently considering candidates interested in Full-Time, Part-Time, or Per Diem opportunities.
The Physical Therapist will evaluate residents’ functional abilities, develop individualized treatment plans, and provide skilled therapy services designed to improve mobility, strength, balance, independence, and overall quality of life.
ResponsibilitiesEvaluate residents and develop individualized physical therapy treatment plans
Provide skilled physical therapy services in accordance with physician orders and residents’ plans of care
Help residents improve mobility, strength, balance, endurance, and functional independence
Monitor resident progress and modify treatment plans as appropriate
Maintain accurate and timely clinical documentation
Educate residents, families, and caregivers regarding treatment plans, safety, and mobility techniques
Collaborate with nursing, rehabilitation, medical providers, and other members of the interdisciplinary team
Participate in care planning and discharge planning as appropriate
Ensure services are provided in accordance with applicable professional, facility, state, and federal standards
Promote a safe, respectful, resident-centered environment
Current Physical Therapist license in the Commonwealth of Massachusetts
Strong clinical, communication, and organizational skills
Ability to work effectively as part of an interdisciplinary team
Experience in skilled nursing, long-term care, rehabilitation, or geriatric care preferred
