Physical Therapist (PT)-$7,500 Sign On Bonus

Woburn Rehabilitation and Nursing Center, part of Stellar Health Group, is seeking a compassionate and dedicated Physical Therapist (PT) to join our energetic therapy team. We offer Full-Time, Part-Time, and PRN opportunities for professionals who are passionate about helping patients improve their mobility, regain independence, and achieve their rehabilitation goals.

Responsibilities
  • Provide physical therapy services that promote excellence in rehabilitation care, interdisciplinary cooperation, and quality patient outcomes.

  • Evaluate and assess patients in a timely manner according to state, payer, and facility requirements.

  • Develop individualized treatment plans and goals based on each patient's needs and provide treatment accordingly.

  • Provide consultations and collaborate with other healthcare professionals to promote optimal patient outcomes.

  • Communicate effectively with staff, patients, families, and other disciplines to ensure coordinated care.

  • Document treatment plans, goals, interventions, and patient progress according to Medicare and other payer guidelines.

  • Participate in family conferences and provide updates regarding patient progress as needed.

  • Complete and submit timely documentation and treatment charges in accordance with facility policies and procedures.

  • Promote patient safety, independence, dignity, and quality of life throughout the rehabilitation process.

Qualifications
  • Current Massachusetts state license as a Physical Therapist.

  • Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.

  • Strong evaluation, assessment, clinical reasoning, and treatment-planning skills.

  • Ability to develop individualized treatment plans based on patient needs and goals.

  • Strong communication and interpersonal skills.

  • Ability to educate, teach, and counsel patients, families, and staff regarding the rehabilitation process.

  • Commitment to providing compassionate, high-quality, patient-centered care.
