Reporting to the Director of Nursing, the Chronic Pain and Addiction Case Manager provides clinical leadership, care coordination, and nursing support for patients managing chronic pain, opioid-related conditions, and substance use disorders. This role combines complex care management for chronic pain, medication-assisted treatment coordination, patient education, and interdisciplinary collaboration to improve patient safety, treatment adherence, recovery outcomes, and overall quality of life.

• Perform comprehensive nursing assessments for patients with chronic pain, including pain history, functional status, medication use, psychosocial needs, and barriers to care.
• Develop, implement, and monitor individualized care management plans focused on improving function, safety, and patient-centered outcomes.
• Coordinate care with primary care providers, pain specialists, behavioral health clinicians, physical therapists, pharmacists, and community resources as needed.
• Provide education on chronic pain conditions, non-opioid pain management strategies, medication safety, coping skills, and self-management techniques.
• Monitor treatment effectiveness, medication adherence, adverse effects, and changes in patient condition.
• Support safe opioid prescribing practices through patient monitoring, risk assessment, care coordination, and provider communication.
• Assist patients with navigating healthcare systems, referrals, insurance barriers, and access to supportive services.
• Track clinical outcomes, follow-up needs, and progress toward individualized goals.
• Screen for high-risk behavior, early recognition and transition to substance use treatment as applicable.

• Provide support to the Addiction Clinical Specialist to assist with management of complex substance use cases and MAT inductions.
• Act as a clinical liaison to the Behavioral Health Recovery Support team.
• Assist in panel monitoring and primary care team support as needed.
