The Department of Mental Health is seeking a Registered Nurse (RN II) to join its Brockton Multi‑Service Center’s PACT (Program of Assertive Community Treatment) team. This role involves delivering compassionate, community‑based psychiatric nursing to adults working toward recovery and independent living. Dutiesand Responsibilities
- Provide wrap‑around services for adult clients by assessing their needs, making home visits, developing supportive relationships, assisting with entitlements, and collaborating with team members and agencies to maximize recovery and independent living.
- Serve on multidisciplinary and individual treatment teams by attending scheduled meetings, presenting current issues, psychosocial histories, mental status, diagnosis, treatment provided, and planned treatment for assigned cases, and participating in discussions to ensure optimal care.
- Assume responsibility for all administrative and clinical record documentation by completing assessments and submitting information in a timely manner to keep treatment providers informed and comply with agency and Joint Commission standards.
- Assist in managing the PACT medication‑management program by transcribing, administering, and recording psychotropic medications, arranging lab work, evaluating effectiveness, complications and side effects, and communicating with the prescriber and team members.
- Assist in organizing medication and medical aspects of client care by conducting nursing assessments, monitoring side effects, collaborating with primary care physicians, and providing education to clients and staff.
- Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals with the treatment team.
- Advocate for clients by serving as liaison between client and community, ensuring appropriate services and prompt on‑site community visits.
- Support and teach clients in daily living tasks, assist with entitlements, navigate legal and social services and network with agencies to maximize independent living.
- Assess and respond to crisis and emergency situations with appropriate clinical treatment, intervention, and contact with supervisors or emergency services.
- Maintain daily and weekly PACT documentation so the team is aware of client needs.
- Involve clients and family in treatment planning, ensuring agreements by signature on a quarterly basis.
- Actively participate in treatment and disposition planning for all assigned hospitalized or incarcerated clients.
- Previous experience working with people with chronic mental illness.
- A current and valid Massachusetts Class D Motor Vehicle Operator’s license or equivalent license.
Applicants must have at least one year of full‑time or equivalent part‑time experience as a registered nurse in a recognized hospital, clinic, or medical facility, or a qualifying combination of experience and education.
