• Assess the health‑care needs of the resident, make a nursing diagnosis, and collaborate with the nursing team to develop an interdisciplinary plan of care that ensures continuity.
• Use a broad scope of knowledge to address the needs of medically complex and mentally compromised residents.
• Establish and maintain functional communication with staff, other disciplines, residents, families, medical staff, and the community.
• Administer medications and treatments according to standards of practice.
• Document care and treatment information accurately and timely, maintaining center records in accordance with state and federal requirements.
• Complete documentation that supports MDS and MMQ processes to ensure center compliance.
• Assess the effect of medications and treatments, monitor these effects, and provide appropriate feedback to the interdisciplinary team and medical managers.
• Lead and direct nursing staff in the safe, effective delivery of care, providing structure and education to residents regarding their health‑care issues and care‑plan strategies.
• Apply knowledge regarding protection of resident rights and treat residents, staff, and the community with respect and dignity; handle resident and facility property and information with honesty, confidentiality, and respect.
• Remain up‑to‑date on all required educational components and pursue skill growth independently, including state CEU requirements.

• Current, unencumbered license as an LPN in the Commonwealth of Massachusetts.
• Proficiency in basic computer skills.
