RN Weekend Supervisor – Long Term Care Facility

Bear Mountain Healthcare has immediate openings for a RN Weekend Supervisor in our Long Term Care Facility in Lowell MA, North of Boston MA.

Job Requirements
  • MA Licensed Registered Nurse license
  • LTC experience required
Essential Duties and Responsibilities
  • Participate with members of interdisciplinary team to plan, coordinate and evaluate residents’ care.
  • Provide direct care and/or assist with care as directed and consistent with scope of practice and competency. Initiate SBAR process and collaborate with RN for assessment and recommendations.
  • Document resident’s condition and nursing needs accurately and in a timely manner. Report pertinent observations and reactions regarding residents promptly to RN.
  • Provide oncoming shift and supervisors with accurate and complete resident status report. Maintain 24-hour report book.
  • Receive and record physicians’ orders.
  • Develop work assignments for Nurse Aides as directed. Provide feedback to Unit Managers regarding performance of Nurse Aides.
  • Report problems and complaints timely to the Unit Manager or Supervisor.
  • Receive and provide accurate and complete resident status report at shift change. Maintain 24‑hour Report Book.
  • Assist in data collection for admission, transfer and discharge of residents and provide the information to the Nurse Manager. Complete clinical admission paperwork. Assist with clinical discharge process.
  • Serve on and participate in committees as assigned.
  • Ensure inventory and supplies maintained in a clean and safe manner. Communicate needs to housekeeping, maintenance or nutritional services.
  • Support and participate in center’s Performance Improvement initiatives.
  • Participate in Survey readiness.
  • Perform rounds with physicians and other team members as needed. Assist physicians and consultants with special tests or procedures within scope of state‑specific nurse practice act and personal clinical competency.
  • Participate in resident assessment and care planning activities, review and revise residents’ assessments and care plans as needed to the extent permitted by state nurse practice act.
  • Coordinate residents’ care activities and communicate changes to other team members and resident family/significant other.
  • Administer treatments.
  • Review, transcribe, communicate and implement physician orders, obtaining RN signature as required by State Regulations.
  • Assist with emergencies, administer cardiopulmonary resuscitation.
  • Document care performed and observations of resident status in the clinical record promptly using standard clinical documentation guidelines required by Bear Mountain and local, state and federal rules and regulations.
  • Monitor completeness and accuracy of own clinical medical record entries and those of assigned LPN and Nurse Aides daily.
  • Participate in medication management to promote optimal safety and effectiveness for residents by:
    • Administering medications as ordered, including next scheduled dose following admission/readmission. Notify Supervisor of medications not available.
    • Observing and reporting resident responses to medication.
    • Identifying and promptly communicating adverse drug reactions.
    • Maintaining narcotic records accurately within scope of practice.
    • Responsible for completing medication interchange and insurance prior to authorization as assigned.
