Pay range: $43 USD to $53 USD. The estimated range is the budgeted amount for this position. Final offers are based on factors including skill set, experience, location, qualifications and other job-related reasons.

Description

Hospice RN Case Manager is responsible for the planning, coordinating, providing and documenting of care of terminally ill patients from day of admission through day of discharge.

MAJOR TASKS,

DUTIES AND RESPONSIBILITIES:

  • Keeps Clinical Director updated on patient status
  • Develops, evaluates, reviews and implements a plan of care for each patient
  • Responsible for the performance of nursing in all areas to facilitate effective, efficient patient care
  • Assures quality care by reviewing and assessing each service
  • Responds to the physical, mental, social and emotional needs of terminally ill patients and their families
  • Responsible for supervising nursing work in cooperation with Director of Hospice Services
  • Provides overall planning, direction, coordination, management and supervision of all nursing and professional services in conjunction with the Director of Hospice Services
  • Is an active member of the Interdisciplinary Team and attends Interdisciplinary Team meetings as scheduled
  • Participates in the On-Call rotation to provide 24/7 coverage for the hospice population

QUALIFICATIONS:

  • Nursing Degree from an accredited college and current RN license in the State of Massachusetts
  • Minimum of 2 years' experience in a clinical setting
  • Experience in program development, implementation and management
  • Experience in community education
  • Excellent organizational skills
  • Ability to work with all members of the Interdisciplinary Team
  • Meets federal and state licensing requirements
  • Ability to communicate in English effectively through verbal and written means
  • Ability to work under stress and in emergency situations
  • Understands principles of growth and development for the entire lifespan with the ability to provide care appropriate for adult and geriatric patients as appropriate to the patient population
Education

Associates or better in Nursing.

Experience

Preferred:
Hospice experience preferred (1-2 years)
