Registered Nurse – Home Health
Shift: 8 hours
Contract Length: 14 weeks Guaranteed

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance
Roles & Responsibilities:
- Assess patients in home health and hospice settings to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and their families on care, medications, and home safety, tailoring teaching methods to their needs.
- Supervise and provide clinical direction to support staff to ensure quality service.
- Monitor compliance with third-party payer requirements for assigned cases.
- Attend interdisciplinary team meetings and provide effective communication among all involved parties.
