Center Well Home Health is seeking a Home Health Registered Nurse to provide one-on-one care in patients' homes, coordinating with physicians and a clinical manager. You will manage care plans, monitor conditions, and educate families to support recovery and independence.
Essential qualifications include an active RN license, CPR certification, and prior med/surg, ICU, ER, or acute care experience. A valid driver's license and reliable transportation are required for travel to patient homes.
