Wellness Nurse

Maplewood Brewster LLC is seeking a compassionate RN or LPN Wellness Nurse to support residents in our assisted living and memory care community in Brewster, MA. In this role, you will provide resident-centered nursing care, conduct assessments, monitor chronic conditions, administer medications, and update care plans. You'll collaborate closely with an interdisciplinary team to enhance quality of life, support safety and wellness, and foster an engaging, supportive environment while growing your skills in a collaborative senior care setting.

Responsibilities

• Deliver comprehensive nursing care to assisted living and memory care residents
• Conduct health assessments, monitor chronic conditions, and update care plans
• Administer medications and treatments in accordance with care plans and regulations
• Collaborate with physicians, families, and care teams to support resident well-being
• Document care accurately and maintain regulatory compliance
• Educate residents, families, and staff on wellness and disease management
• Respond to urgent health concerns and participate in emergency procedures
• Support a resident-centered, compassionate care culture
