MDS Coordinator $7500 Sign on Bonus

We are seeking a dedicated and experienced MDS Coordinator to join our dynamic healthcare team at Woburn Rehabilitation and Nursing Center.

As a critical member of our interdisciplinary team, you will play a vital role in coordinating resident assessments, developing comprehensive care plans, and ensuring high-quality, resident-centered care and regulatory compliance. • Conduct and coordinate the development and completion of comprehensive resident assessments
• Conduct resident assessments in accordance with current state and federal rules, regulations, and guidelines
• Implement resident Care Area Assessments (CAAs)
• Coordinate and support the development and implementation of individualized resident care plans
• Collaborate with the interdisciplinary care team to develop comprehensive assessments and care plans
• Participate in facility survey readiness and throughout the survey process
• Ensure MDS-related documentation is accurate, complete, and submitted within required timelines
• Communicate effectively with nursing staff, physicians, therapists, social workers, and other interdisciplinary team members
• Support compliance with applicable regulatory requirements and facility policies

• Current, unencumbered Massachusetts RN or LPN license
• Minimum of two years of MDS and PPS experience
• Previous care planning experience required
• Strong assessment, documentation, and organizational skills
• Excellent communication and interpersonal skills
• Ability to work effectively with an interdisciplinary team
• Strong knowledge of applicable MDS, PPS, and regulatory requirements
• Commitment to providing high-quality, resident-centered care

• Competitive MDS nursing wages: $45-$55 per hour
