Job DescriptionThe Child Youth and Family Services operated by the Department of Mental Health at the Brockton Site Office seeks a dynamic candidate to provide Case Management services for eligible youth under 22, with majority of caseload below 18. The successful candidate will work with youth with mental health needs and their families to promote recovery and community-based services in a trauma‑informed, person‑centered approach.
Duties and Responsibilities- Provide case management services for eligible youth under 22, with majority of caseload below 18, engaging families/guardians in the CYF Family Collaboration Principles.
- Develop creative engagement strategies and maintain regular contact to sustain goals and community tenure.
- Collaborate with internal and external providers and families to ensure integrated planning and service provision.
- Prepare and maintain documentation per DMH regulations and EHR rules, including progress notes.
- Conduct scheduled visits at client home or residence and monitor safety.
- Coordinate with multi-service/system teams (family, school, state, community) to facilitate and monitor service implementation.
- Plan, convene, and chair care‑planning team meetings to review individual service plans and coordinate delivery.
- Knowledge of psychosocial problems and mental/emotional disorders.
- Knowledge of counseling, interviewing, crisis intervention, motivational techniques.
- Familiarity with programs and services available to the mentally ill population.
- Understanding of laws, rules, policies governing agency activities.
- Ability to conduct case assessments through records, questioning, and observation.
- Strong oral and written communication skills.
- Decision‑making capability, calm demeanor in stressful or emergency situations.
Travel is required across the DMH Southeast Area and occasionally statewide. A current and valid Massachusetts Class D (or equivalent) driver’s license is required.
Preferred Qualifications- Master’s degree in human services, psychology or social work.
- 3+ years of experience in child/adolescent behavioral health services.
- Experience working with youth and families in community and home‑based settings.
Salary range: $68,435.12 – $93,470.00 per year.
