Case Manager, Askia

You build trusting relationships, navigate complex systems, and help individuals overcome barriers to stable housing, healthcare, employment, benefits, and community support. The Dimock Center is a federally qualified community health center that heals and uplifts individuals, families, and communities. Our vision is to redefine the model of a healthy community by creating equitable access to comprehensive health and wellness services. ASKIA is a co-occurring enhanced residential rehabilitation program serving adult males with substance use and behavioral health needs. As a Case Manager, you will be an integral member of the clinical team, providing individualized care coordination, advocacy, resource navigation, and discharge planning to help clients build the foundation for successful recovery and community reintegration.

Key Responsibilities
  • Build and maintain trusting, professional relationships with clients while engaging them in treatment and recovery goals.
  • Meet individually with clients on a regular basis to assess needs, coordinate services, monitor progress, and support aftercare planning.
  • Conduct comprehensive assessments to identify client strengths, needs, barriers, and available supports.
  • Develop and implement individualized care plans in collaboration with clients and the interdisciplinary treatment team.
  • Coordinate referrals and link clients to medical, behavioral health, substance use, housing, employment, financial, legal, and community-based resources.
  • Advocate for clients and help them navigate complex health and human service systems.
  • Identify and address whole-health needs and social determinants of health that may affect treatment, recovery, and successful community transition.
  • Coordinate care with primary care providers, medical and behavioral health specialists, community providers, courts, and other external partners.
  • Support clients with applications and processes related to housing, employment, benefits, financial assistance, and other essential resources.
  • Coordinate transportation and accompany clients to court, medical appointments, community meetings, and other required appointments as appropriate.
  • Facilitate psycho educational and recovery-focused groups.
  • Partner closely with counselors, clinicians, recovery specialists, and other members of the treatment team to promote progress toward treatment plan goals.
  • Begin discharge and aftercare planning well in advance of discharge to ensure continuity of care and appropriate community supports.
  • Conduct post-discharge follow-up and assist clients in maintaining connections to services and supports.
  • Assist the clinical team with crisis intervention and management as needed.
  • Maintain working knowledge of Dimock's continuum of care and connect clients with appropriate internal and external resources.
  • Complete all clinical and administrative documentation accurately, thoroughly, and within required timelines in accordance with Dimock, Joint Commission, regulatory, and contractual standards.
  • Respond to calls and requests from external providers in a timely and professional manner.
  • Attend staff meetings, trainings, case conferences, and other required meetings.
  • Maintain appropriate professional boundaries and protect the confidentiality and dignity of clients, families, visitors, and coworkers.
  • Maintain effective, respectful working relationships across Dimock and with community partners.
  • Follow all organizational policies, procedures, applicable federal and state regulations, and contractual requirements.
  • Perform other duties as assigned.
What You Bring
  • Bachelor's degree in human services, psychology, sociology, health services, or a related field.
  • At least one year of experience working in a substance use treatment program.
  • Experience working with individuals experiencing substance use and/or mental health challenges, homelessness, or other complex social needs preferred.
  • Strong verbal and written communication skills.
  • Strong interpersonal skills and the ability to build rapport with individuals from diverse backgrounds.
  • Ability to navigate community resources, coordinate multiple services, and advocate effectively on behalf of clients.
  • Strong organizational skills with the ability to prioritize competing responsibilities and meet deadlines.
  • Ability to exercise sound judgment when managing difficult, emotional, or crisis situations.
  • Ability to work effectively as part of a multidisciplinary team while also taking initiative and working independently.
  • Ability to communicate and represent Dimock professionally within the organization and throughout the community.
  • Intermediate computer skills, including Microsoft Word, Excel, and PowerPoint.
