Location: Boxborough

The Case Manager is responsible for assessing, planning, coordinating, and facilitating services for clients, including individuals with disabilities, the elderly, and those with special needs. The Case Manager acts as a primary advocate for clients, ensuring they receive the support, resources, and services required to enhance their quality of life. This role involves building relationships with clients, conducting assessments, coordinating care plans, and maintaining regular communication to ensure clients’ needs are met effectively.

Responsibilities
  • Conduct client assessments and develop individualized care plans
  • Coordinate services with healthcare providers, community organizations, and family
  • members
  • Connect clients with resources, including healthcare, housing, transportation, and social
  • services
  • Monitor client progress and update care plans as needs change
  • Advocate for clients and help them navigate healthcare and social service systems
  • Maintain accurate case records, documentation, and reports in compliance with agency
  • policies and HIPAA regulations
  • Conduct regular follow-up visits and provide ongoing support to clients and caregivers
  • Collaborate with interdisciplinary teams to ensure continuity and quality of care
Qualifications
  • Bachelor's degree in social work, Human Services, Psychology, or a related field
  • Minimum of two (2) years of experience in case management, social work, or a related
  • healthcare or human services setting
  • Strong communication, organizational, problem-solving, and interpersonal skills
  • Knowledge of community resources and case management practices
  • Proficiency with Microsoft Office and electronic documentation systems
  • Valid driver's license and reliable transportation
  • Licensed Social Worker (LSW) or Case Management Certification (CCM) is preferred
