What You'll Do at DBH:
Performs psychosocial interventions with clients Performs individual, family or group intervention (e.g., counseling, & education of the client) Develops intervention goals & objectives designed to give education & resolve client's problems in functioning Participates in process of establishing, implementing & evaluating treatment system's goals & objectives Interviews clients & client's family to establish case goals & objectives Assesses clients & determines specific intervention techniques for clients & monitors client's responses to treatment systems & provides feedback on effectiveness of prescribed treatment, evaluates client's progress resulting from prescribed treatment plans.
Plans & implements continuity of care from hospital to community by functioning as linkage for treatment team with case management system through ongoing regular collaboration (e.g., meetings, telephone conversations, correspondence). Maintains necessary data documents to satisfy requirements of federal, state & local agencies, completes & completes forms required by law &/or department policy. Serves on treatment teams & program development teams, attends training meetings, & intradepartmental meetings.
Establishes & maintains working relationship with other facilities, agencies & support services in order to coordinate referrals & conduct follow-up services. This is an hourly position covered by the Ohio Health Care SEIU/1199 bargaining unit (union), with a pay range of 10 or 11 (dependent upon on licensure as a LSW or LISW) on the Ohio Health Care SEIU/1199 Pay Range Schedule .
Normal working hours are Monday
- Friday, 8:00a-4:30p. This position is located within our Heartland Behavioral Healthcare campus at 3000 Erie Street South, Massillon, OH 44646 . Unless required by any applicable union contract and/or requirements of the Ohio Revised Code, the selected candidate will begin at Step 1 of the pay range schedule listed above, with an opportunity for pay increase after six months of satisfactory performance and then a yearly raise thereafter.
