Job Title
Home Health RN Case Manager (RNCM)
Schedule & Service Area- Part‑time RNCM:
Minimum travel radius of Stark County up to 60 minutes. - Full‑time RNCM:
Includes Stark, Summit, Wayne, Tuscarawas, Columbiana, Carroll, and adjacent counties up to a 2‑hour radius. - Applicants may reside in or near any primary service area as long as travel requirements are met.
- Flexible and customizable. Schedules built to fit availability and preferences.
- Minimum availability required:
Monday to Friday, 8 a.m. – 5 p.m.
- No on‑call or overnight shifts.
- Weekend services are optional.
- Overtime, respite, and PRN services are offered when available but are not required.
- Long‑term employment position requiring a recurring weekly schedule.
Home Care Network, Inc. (HCN) is a nurse‑owned, locally led home health agency dedicated to patient care and employee wellbeing.
Role Overview- Case management, start of care (SOC), recertifications, resumption of care (ROC), and other assessments and services.
- Occasional new hire orientations, in‑service and supervisory reviews.
- Back‑up for occasional skilled nursing visits across most areas.
- Travel to multiple counties.
- Opportunities for career advancement to leadership positions.
- Perform client assessments, plan and initiate appropriate actions in home‑care settings.
- Manage and supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- Document, communicate, and coordinate patient status changes to the care team.
- Promote personal safety and a safe environment for clients and coworkers.
- Travel surrounding counties as required.
- Maintain at least one year of nursing supervisory experience; home‑care experience preferred.
- Graduate of an accredited School of Nursing.
- Minimum of one year experience as a Registered Nurse with an active RN license in the State of Ohio.
- CPR and First Aid certifications required.
- One year of case‑management experience, with home‑health experience highly preferred.
- Experience in supervisory role, wound care, lab draws, and OASIS assessments is a plus.
- Strong background in nursing, patient care, and confidentiality.
- Strong attendance and dependability record.
- Adherence to HIPAA regulations and medical terminology.
- Current driver’s license and reliable transportation.
- Desirable traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, respectful, and positive.
- Part‑time hourly wage: $35 – $40 per hour.
- Full‑time salary range: $72,800 – $83,200 per year, with quarterly bonus opportunities. (Requires pre‑determined minimum visits/services per week.)
- Round‑trip mileage reimbursement.
- Flexible schedule, supported by an in‑house scheduling team.
- Quality and compliance support and training.
