What are YOU looking for in your next nursing position?- Community Health Network, LTD (CHN) is looking for a Licensed Practical Nurse (LPN) OR Registered Nurse (RN) who wants to
- 1) provide care to someone in their home,
- 2) maintain a consistent caseload
- 3) work closely with an Administrative Team that appreciates them.
Homecare Nursing requires strong communication but promotes growth and independence. No productivity levels or 45 bed assignments!

- Medication administration, updating orders and day-to-day care coordination.
- Effectively communicate with supervisors and guardians
- Dietary support and monitoring
- Relevant education and training based on the individual's care plan
- Complete all other duties as assigned
- Certificate in nursing from an accredited nursing school
- Licensed as a LPN/RN as issued by the State of Ohio-Registered on the State Nurse Registry
- BLS CPR
- High school diploma, High school transcripts, GED or equivalent
- Driver's License
- Two professional reference letters
- Social Security Card
