Home Health Aide (HHA)

As a Home Health Aide (HHA) with SYNERGY Home Care of Woodbridge, you will play a vital role in delivering high-quality care to patients in their own homes. Our team is dedicated to providing compassionate and personalized support to individuals in need, and we're looking for a compassionate and skilled HHA to join our family.

Responsibilities:

• Provide personal care services such as bathing, dressing, and grooming to patients in their homes
• Assist patients with daily living activities, including meal preparation, light housekeeping, and medication reminders
• Monitor and report changes in patient condition to healthcare professionals
• Maintain accurate and detailed records of patient care and services provided
• Collaborate with healthcare teams to develop and implement patient care plans
• Demonstrate empathy and understanding towards patients and their families, promoting a positive and supportive environment

Requirements:

• High school diploma or equivalent required
• Current certification as a Home Health Aide (HHA) in the state of New Jersey
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Ability to lift, transfer, and move patients safely and comfortably
• Valid driver's license (if applicable)
