Part-time Social Worker (LSW / CSW / LCSW) – Skilled Nursing & Rehab Make a Meaningful Difference Every Day
Complete Care at Madison is seeking an experienced and compassionate Social Worker (LSW / CSW / LCSW) to join our skilled nursing and rehab team.

We are looking for a social worker with long-term care and rehabilitation experience who is confident navigating admissions, discharge planning, family support, and interdisciplinary care coordination.

- Coordinate admissions, discharges, and transition planning
- Provide emotional support and advocacy for residents and families
- Participate in interdisciplinary care plan meetings
- Educate residents and families regarding Advanced Directives
- Maintain accurate documentation in compliance with state and federal regulations
- Assist with discharge planning, community resources, and post-acute coordination
- Support residents through rehabilitation stays and long-term care placement
- Current New Jersey LSW, CSW, or LCSW license required
- Minimum 1–2 years of experience in a healthcare setting, preferably skilled nursing, long-term care, or rehab
- Experience with discharge planning, Medicaid/Medicare, and MLTS preferred
- Understanding of CMS regulations and Department of Health survey process preferred
- Strong communication, documentation, and organizational skills
- Ability to work independently while collaborating with an interdisciplinary team
