At Mattapan Health & Rehabilitation Center, our Social Workers play a vital role in creating a supportive environment where residents feel heard, respected, and empowered. As a member of our interdisciplinary care team, you will help residents navigate life transitions, advocate for their needs, and support their emotional, social, and psychological well-being.
Our 85-bed skilled nursing and rehabilitation community provides both Short-Term Rehabilitation and Long-Term Care, giving our Social Workers the opportunity to support residents throughout a variety of healthcare journeys.
This role goes beyond paperwork and care coordination — it is about building relationships, protecting resident dignity, and helping individuals maintain the highest possible quality of life throughout their time at Mattapan.
