Care Coordinator SupervisorThe Care Coordinator Supervisor provides leadership and supervision to care coordinators, volunteers, and interns while delivering direct services to complex elderly clients, resolving escalated issues, ensuring accurate and timely documentation and reporting, supporting staff development, collaborating with internal partners, and traveling within assigned communities to fulfill program needs.
Job Responsibilities- Provide direct individual and/or group supervision for care coordinators, volunteers and interns; provide direct service to elderly and frail clients with complex issues. Resolve complex complaints from clients and community partners.
- Responsible for assigning activities to and training of care coordinators, case associates and other administrative staff. Ensure staff activities are performed in an efficient, prompt, professional and accurate manner.
- Audit and maintain up-to-date and accurate client files and databases. Complete monthly reports, collect, analyze, and maintain data to comply with established time frames and requirements. Required to travel independently throughout assigned communities to complete position duties as needed.
- Work collaboratively with internal stakeholders and team members as needed.
- Supervise and support assigned staff by setting clear expectations, providing guidance and feedback, fostering accountability, and cultivating a collaborative, positive, and purpose-driven team environment.
- Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.

- Bachelor's Degree in Arts/Sciences (BA/BS) 3-5 years in a related field of study
- Master's degree 5+ years in a related field of study
