Position: Family Medicine or Adult Medicine opportunity
Type: 100% Outpatient
Call: 1:5 telephone only, and supported by nurse advisory line
Model: Patient Center Medical Home Model
Start date: 60-90 Days
Schedule: Mon - Fri with flexible scheduling
Job Description: Typical outpatient Primary Care services
Patient Population: All Ages or Adult Medicine
Patient Load: 18-22 PPD
EMR: Epic
Support: 2 experienced APPs Full admin staff
