Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team, providing comprehensive nursing services in both outpatient and hospital settings. The ideal candidate will demonstrate expertise in nursing practices and a strong commitment to patient care.
Responsibilities- Conduct thorough patient assessments and triage based on medical needs, ensuring timely and effective care delivery.
- Collaborate with interdisciplinary teams to enhance patient outcomes, including hospice palliative medicine when applicable, fostering a culture of collaboration and care.
- Develop and implement individualized care plans, prioritizing patient-centered care and promoting positive health outcomes.
- Provide direct patient care, including administering medications, performing venipuncture, and managing medical documentation, ensuring accuracy and attention to detail.
- Educate patients and families on health conditions, treatment plans, and preventive measures, empowering them to take an active role in their healthcare.
- Monitor patient progress, adjusting care plans as needed, and maintaining accurate medical records while ensuring compliance with healthcare regulations.
- Participate in case management activities, coordinating care across multiple services to ensure seamless patient transitions and optimal outcomes.
- Current RN licensure is required, with a strong foundation in nursing practices and patient care.
- Experience in skilled nursing care is preferred, with a focus on delivering high-quality patient care.
- Familiarity with medical terminology and documentation practices is essential, with a commitment to accuracy and attention to detail.
- Strong communication skills and the ability to work effectively in a team environment are necessary, with a focus on collaboration and patient-centered care.
