We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care and a focus on delivering high‑quality medical services. This role requires excellent communication skills, attention to detail, and the ability to collaborate within a multidisciplinary team.

• Providing direct patient care including administering medications and treatments as prescribed by physicians
• Documenting patient information accurately in EMR and maintaining compliance with healthcare regulations
• Monitoring patients' vital signs and reporting any changes to the healthcare team
• Working closely with physicians, care team partners, residents, and family members

• Qualified candidates should have a valid LPN license and relevant clinical experience in a healthcare setting
• Strong understanding of medical documentation practices and ability to navigate electronic health record systems
• Excellent interpersonal skills to foster positive relationships with patients, families, and team members
