POSITION TITLE: SSVF Case Manager – Supportive Services for Veteran Families (SSVF) Program – Mattoon/IL-515 CoC

LOCATION/DEPT: Supportive Services for Veteran Families (SSVF Program) – IL-515 Mattoon, IL

REPORTS to (TITLE): SSVF Region 2 Supervisor

FLSA CATEGORY: Non-Exempt

STATUS TYPE: Regular, Full Time

PAY GRADE: 205 – Grant Funded

PAY RATE: $19.97 - $23.96 Hourly

OUTCOMES

NSI Supportive Services for Veteran Families (SSVF) program aims to provide critical time intervention, strength-based case management approach to Veterans and their families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives through sustainable permanent housing interventions and foster a deeper understanding of the life-changing work carried out by The Salvation Army North and Central Division.

ESSENTIAL

DUTIES
  • Case Management (70%)
  • Ensure program criteria, referrals and procedures are maintained for SSVF program Veteran participants.
  • Develop a case plan with the participant within one week of enrollment with specific short- and long-term goals, when needed.
  • Schedule regular meetings with participants to review and update goals. Conduct home visits as appropriate.
  • Provide participants linkages to community services as needed.
  • Provide services as needed, eg., court appointments, landlord/tenant conflict resolution, etc.
  • Educate Veterans in life skills such as Resiliency, Tenant Education, and Financial Literacy.
  • Participate in housing searches and scheduling housing inspections.
  • Provide intake including required documentation for admission, referral needs, program details and expectations.
  • Conduct outreach, intake, and enrollment activities.
  • Assist Veterans in filing for SSDI/SSI benefits through SSA’s SOAR certification program.
  • Using strengths-based perspective, encourage positive lifestyle changes for Veterans to maintain housing.
  • Provide resources and referrals as needed to Veterans and household members.
  • Flex schedule to meet with full time employed Veterans during non-standard business hours.
  • Ensure client confidentiality in accordance with established procedures and regulations.
  • Perform other duties as assigned related to the mission and goals of the SSVF Program.
  • Administrative Responsibilities (20%)
  • Maintain complete and accurate electronic client records that includes the assessment, eligibility, intake, case plan, documentation of progress, challenges, accomplishments, discharge plan and all follow up contact.
  • Provide electronic detailed case note records.
  • Participate in quality assurance/CARF Task Force Group.
  • Attend community meetings and Coordinated Entry By-Name-List reviews.
  • Attend community Veteran events such as Stand Down, Veteran Resource Fairs, etc.
  • Communicate effectively with VA HUD-VASH and Salvation Army personnel.
  • Attend weekly, monthly meetings and annual training events.
TRAVEL
  • Travel throughout the program’s service area is required on a regular basis to meet with Veterans, community partners and to attend mandatory meeting events.
EDUCATION/EXPERIENCE
  • Bachelor’s degree in human services, social work, or a closely related field.
  • One year of direct, full-time case management experience working with the homeless population and/or working with Veterans.
  • An in-depth knowledge of VA programs and guidelines.
COMPENTENCIES
  • Team player, self-starter, and perform well with minimal supervision.
  • Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
  • Must be able to work in diverse environments such as homeless shelters, service facilities, streets, offices, hospitals and all other locations as necessary to fulfill program objectives.
  • Experience in working with Veteran population and have experience in areas of substance abuse, Veterans assistance and PTSD is beneficial.
  • Prior knowledge and training in Critical Time Intervention (CTI), harm reduction, Housing First, Trauma Informed Care, motivational interviewing and conflict resolution is preferred.
