Transitions Care is seeking a compassionate Social Worker/LSW to support hospice and palliative patients and their families. In this role, you will complete psychosocial assessments, provide counseling and crisis intervention, coordinate community resources, and guide families through goals-of-care and end-of-life decisions. You'll collaborate closely with an interdisciplinary team, contribute to care planning, and document in the EMR. This position offers the opportunity to grow your clinical skills in a mission-driven, patient- and family-centered environment focused on dignity, comfort, and quality of life.
Responsibilities
- Provide psychosocial assessments for hospice and palliative patients and families.
- Develop and implement individualized care plans addressing social and emotional needs.
- Offer counseling, crisis intervention, and grief support to patients and caregivers.
- Educate families on hospice philosophy, goals of care, and available resources.
- Coordinate community resources, benefits, and referrals to support patient needs.
- Collaborate with the interdisciplinary team on care planning and family meetings.
- Advocate for patient rights, preferences, and advance care planning.
- Maintain timely, accurate documentation in the electronic medical record.
- Participate in on-call rotation and home, facility, or hospital visits as needed.
- Support bereavement services and follow-up for families after loss.
